FEES

All services are 45-60 minutes in length, unless other arrangements are made with the therapist.

Initial intake assessment       $ 200.00
Individual therapy                   $ 170.00
Family/ couple therapy         $ 180.00

All co payment deductibles are due at the time of the session. If you are not using insurance then the
full session fee is due at the time of the session. If you are using health insurance, we will submit your
claim directly as long as your company responds in a timely manner (within 30 days). Please understand
that you are responsible for payment should your insurance fail to pay. Please be aware that insurance
companies do not reimburse all issues, conditions, and problems that are the focus of therapy. It is your
responsibility to understand and verify the specifics of your insurance coverage, as you are responsible
for full payment if your insurance rejects your claim. There are charges for special services such as
copying records, preparing summaries, comprehensive written reports, phone consultation, or
court appearance testimony. Please discuss with Dr. Beyzavi.

INSURANCE

Medical Assistance, Ucare, Medica, blueCross/Blueshield, Health Partners,
Metropolitan Health Plan, America's PPo, HealthEZ, Cigna, Preferred one, South County
Health Alliance, and most private insurance.

CANCELLATION POLICY

We ask for a minimum of 24 hours notice if you wish to cancel or reschedule an appointment,
since the scheduling of your appointment involves the reservation of time specifically for you. There is a
charge of $ 50 for missed appointments, which is your responsibility and is not reimbursed by insurance
companies. In cases of appointment cancellations due to emergency or sudden illness, this policy will
not be enforced.
